Who is responsible

IReAdapt Vision is the vision rehabilitation and assistive-technology service division of IReAdapt LLC. Questions may be sent to info@ireadapt.com.

Information collected

Depending on how you use the site, this may include contact details, account information, service-plan status, appointment or support-time records, purchased resources, newsletter consent, and messages you submit. The site also stores security records such as a one-way hash of an Internet address, sign-in attempts and administrative audit events.

Payments

Online payments and subscriptions are processed on Stripe-hosted pages. IReAdapt Vision stores transaction identifiers, status and amounts needed to administer access and billing, but this website does not receive or store complete card numbers, CVCs or bank-account credentials.

How information is used

Information is used to respond to inquiries, provide and document services, manage accounts and subscriptions, deliver purchased resources, send requested communications, prevent abuse and meet business or legal obligations. Information is not sold.

Service notes and sensitive information

Do not submit medical records, student records, Social Security numbers, passwords or payment information through general forms. Member and administrative notes should contain only the minimum information necessary for service administration.

Providers and international access

Hosting, email and payment providers process information to operate this service. Remote services may be offered worldwide where lawful and practical; information may therefore be processed in the United States.

Retention and choices

Records are kept only as reasonably necessary for service delivery, security, accounting and legal obligations. You may unsubscribe, request correction of account information, or ask a privacy question by contacting info@ireadapt.com.
